Then you've got to get a staff together which if you've got the money to buy or lease a whole heap of planes (even if secondhand) shouldn't be too much of a problem, you'll need pilots and flight attendants but you'll also need the people to run your ticket counters and to run the day to day functions of the business. Maintenance personnel may also be needed although you can outsource maintenance (but keep a close eye on whoever you outsource to and it may be cheaper to do maintenance in house if you have a lot of aircraft (you want a major airline right?)).

Then you've got to get permission to land at airports which should be possible, major airports with a lot of slot limitations might not let you land there but smaller airports should not be too much of a problem and most of them will still serve a lot of passengers and have much less delay anyway. International flights might not be possible from the less major airports but if you've just started out you won't be doing any international flights for a while.
Then you have to convince the government regulators that your airline will be safe. If you get good equipment, have a good training programme for your staff and make sure your maintenance is up to scratch you should be able to convince them to let you fly (you might manage it even if things aren't safe but I don't think you should even consider that course of action). Getting ETOPS approval will take a while but you won't be making any ETOPS flights for a while anyway.
