Depends upon what your aim is and what do you want to say about yoursel? If you're looking for the California Surfer Girl, you should add a lei. If you want something "Goth" you could hang a shrunken head on the rear view. Something Holy? A Rosary or Guardian Angel visor clip. A sunscreen in various designs adds to the car's appeal (and protects the interior from the sun's rays); you can also have the top part of the windshield covered with a sun screen or glare blocker (available at all auto supply stores, WalMarts, etc).

As I said, it is entirely up to your imagination. My PT Cruiser (Inferno Red) has red dice, a Rosary and my Handicapped Plaque on the rear view mirror. Other than that, it's basically stiock (temporarily); as soon as I can afford it I'm getting a new chrome grille and some other exterior stuff.

I saw one car in a parking lot on a trip that had a soft ball sticker
thing that was half a soft ball(or base ball) with marks that made it look like some one had through a ball at the window. Hope that helps.
