Question:
On open roads or on level roads it does well It appears to lose air or lack of fuel what do you think the problem is?

Answers:

How is the octane tested in gasoline and is there a way for the public to test what they purchase?

Take it to a Ford dealer, or mechanic experienced in diagnosing diesels. Have him/her check your MAP/EBP/BARO readings to ensure their accuracy. They are the sensors that affect turbo boost, especially EBP. If the EBP tube is plugged, or the sensor is bad, that will certainly cause a lack of power concern. Pay particular attention to your MAP and BARO readings as well. All that, and make sure the air filter is clean, as well as the fuel filter. Hope this helps.

Can i get a CDL if i had a DWAI in the past?

Oxygen is thin at over 8500 feet. The engine and the turbo charger need oxygen to produce power. Also, your truck only produced 210 hp when it was new. Being over a decade old, it's probably producing around 150 at the rear tires. That's a lot of truck to be pulling, especially with a trailor. Your diesel was built to produce torque at a low RPMs instead of HP.

How much smoke does a 180 hp engine generates?

other things to consider is your ratio from transmission to your duals. Some are geared to run on flat land, others are geared towards climbing hills!
