The clutch assembly consists of three components. The cover plate or pressure plate. The disc, driven plate, centre plate or clutch plate and the release or thrust bearing. The first bolts onto the engine flywheel and is spring loaded and can be released by pressing the pedal which by cable or hydraulics presses the release bearing on it, so that the driven plate, which has friction linings on it, can run free or slip if not fully released between flywheel and pressure plate. Deliberately letting the clutch partially engage or using it to hold the vehicle on a hill excessively can wear the linings away which will eventually cause slip when the pedal is not depressed. Keep running the vehicle and the fixings for the linings will ruin the pressure plate and flywheel. When the driven plate gets old , even if it works Ok, the clutch engagement is less smooth. If the springs in the pressure plate go you will feel it's eccentric action through the pedal and it could soon fly apart. If the thrust bearing is faulty it will make a noise, supressed in the early stages by pressing the pedal slightly. If you make a habit of holding the clutch pedal down instead of selecting neutral you will shorten the life of the pressure plate and release bearing.If the vehicle has done say 70000 miles or more and anything fails I would replace the lot, because all components are arguably at the end of their design life. When I refer to the "Clutch" I am commenting on the whole unit made up of many parts, from the foot control to the thrust Bearing. the Clutch may be operated by Hydraulics or a series of rods or cables depending on age of you Vehicle.(Classic Cars eg)The main parts that are generally replaced, are the pressure plate. Clutch plate and the afore mentioned thrust Bearing.
Most Macanics would just replace the Clutch Plate but would closely inspect the other working parts while the pressure plate etc were exposed.
