What causes potholes in the streets?

I've been driving home from my job lately and notice that I've been dodging a few potholes. And I always wonder to myselve how do these potholes exist.

Water gets under the road. Then it freezes and expands. Then it melts, leaving a hollow spot. Then someone drives over it and caves in the blacktop. Then more people drive over the hole, expanding it further.

My drivers permit has expired?

to add to the one above, it doesnt help when snow plows chip away at it too (then you get peppered with asphault *shakes fist at ODOT*)
