Does depend a little on the boat, but not much.From the engine exhaust manifold you need marine exhaust hose. Lead it DOWN (as low as you can make it) to a waterlock (any chandler will tell you what one is), it is very important though. A waterlock will prevent sea water siphoning back into the engine when you turn it off.
Next, from the other end of the waterlock, run more exhaust hose leading UP to the transom (or wherever you want to site the exhaust outlet). You should put a loop in this section of hose so that it runs up above the outlet site and curls down to it. When sea water enters the exhaust outlet from outside the boat, it will only be able to penetrate to the the upward bend and no further.
At this point you need only to connect the exhaust hose to the outlet.
Drill a hull through the hull where you want the exhaust to run out and place a through-hull fitting in the hole. Sometimes known as a "skin fitting", you push it in from the outside and lock it in place with the nut that is supplied with it. Use sikaflex (marine sealant) to seal the fitting in the hole.
There are dedicated through-hull fittings for exhausts, pick one you like but stainless steel is best.
The last thing to point out is that you should connect each section of hose with TWO jubilee clips at each connection point.
