You should submit that form and all required documentation (as specified in the form) to a local Florida Tax Collector's Office or the Florida Tax Collector's office located in your county of residence for processing. In most cases, the Department of Motor Vehicles will also require that you get a Lost Motor Vehicle Title Bond. Call your local or state DMV or call your local Tax Collector's office to see if you are required to be bonded for your specific case. No you have to go to your local DMV and fill out a title application form (don't know if you have to fill out a form in Florida,but I do know that you have to fill one out in Texas).
