What is a good point of sales contract to give someone who is buying a used car from you?

Basicly it should include the buyers and sellers name,VIN # and the price sold for..Also should state that it is As-IS,if that is the case..Or list any warranty that it has..Would be a much stronger contract if it was notorized..
