As for which is better, it depends on how you install the Starmate. If you have a satellite ready stereo in your car and choose to hardwire the Starmate into that, your quality will be about the same as a strong HD signal. If you are using the Starmate's FM transmitter, then the quality will be significantly lower because the FM transmitter sends a standard analog signal rather than a digital signal. Many new car stereo decks are both satellite ready and HD though, so you really could have both, although you can get good quality satellite ready, non-HD decks real cheap.

My personal recommendation for now at least is to go with the Starmate and have it hardwired into your car's stereo system, because most regular radio stations haven't upgraded their systems to HD yet. However, with both XM and Sirius competing with standard radio, this will change over the next few years and digital HD will soon be the standard.
The bottom line is its up to you. The Sirius will involve a subscription fee, but you can get a satellite ready, non-HD deck for pretty cheap. With the HD, you are going to pay a little more for the deck and your selection of HD radio stations will at least initially be limited, but there is no subscription fee. Its also worth noting that the Sirius is commercial free on all of its music stations, so if commercials annoy you, that is a huge plus.
