Make sure your speakers match the impedance of the output. keep in mind that if you wire speakers in parallel that you will reduce the total impedance for that circuit. Example (2) 4 ohm speakers wired in parallel will give you a total of 2 ohms for that circuit and if you hook up that circuit to an amp that is set for 4 ohms you will be drawing twice as much current and generating twice as much heat.

Well, its been a while since I have installed one so going on memory here. Is it a stereo sub or is it mono. If it's mono there should be a switch on the amp that needs to be set to sub. Make sure that is correct and also check the impedance of the amp and sub. They should match. If not some amps have a switch for this, or they may do it automatically(look in owners guide).

After reading your edit. I think you have blown your sub. Possibly a short in it or an enternal crossover if it has one. So, you may have to replace your sub.
