Too much tention on the belt, seized or siezing pulley, bhur or marking on a pulley, pulley out of align from the rest. There are may possibilties. I would be guessing the tention would be the culpret. A general rule of them i go with is if you can rotate the belt 180degree at the longest point in the belt while the belt is still holding tention, but not so much that you're stuggling to turn it, It would usually be good. When you have the belt off, spin all the pullies, if any don't move (with exception of the crank pulley) in the alternator belt line, or if any are making noises that are not out of the ordinary then they would probably need replacement. Also when the belt is off, inspect the surface area of the pullies making sure there are no markings on it and it's smooth. Now with the belt on, start the car (and keep it in park, or neutral with the park brake on if its a standard) and double check the work, make sure the belt is fully on all pullies, and make sure the belt is not "wobbling" and that its straight.Most likely bad pulley alignment, or the belts are too tight.

I'm not familiar with the tensioning setup on that vehicle, but if there is an idler or tensioner in the belt loop, the bearings in it could be loose/worn or even seized...that condition will cause premature belt wear/failure.
