Why did this happen?

Question:
The pilot bearing was completely wore out, but the input shaft bearing was fine. the plate that holds the bearing in the tranny, and has the sleeve that goes into the throw out bearing was completely back out and in the bell housing. all four bolts were backed out. I assume the pilot bearing wore out and rattled the sleeve and plate loose, but i never felt an extreme rattle. and the bolts had thread sealant. on them.

You would only feel the rattle when you shifted . Once the clutch was ingauged the disk would be stable .
