I work for Lexus, but am not a technician. If I am correct the light is easiest to install from underneath the car. We have had several customers come into have the light installed because they had failed. The dealer usually lifts the car up in the air and has a better angle to work at. We typically charge $40-50 per light bulb (excluding headlights which are near $300-400 for some HID bulbs) The $40-50 includes the bulb and the installation. For my money it would not be worth the trouble of fussing with it myself.
