I can't tell you exactly what it might be, but here are a few things to check out. Might consider changing the fuel filter once or twice a year, make sure the spark plugs and plug wires are good. The fuel filter can cause poor fuel mileage and even rough idling or running if it is clogged or dirty, and the plugs of course can cause it to run poorly if they are bad. If those two simple things don't fix the problem then it could be something else. Also, don't mess with the sparkplugs yourself! They can get cross threaded in the 5.4 so easily and then it's on to big bucks. Let the dealership handle the plugs. Doing the maintaince such as cleaning the throttle body and such is a wise move as it does effect idle quality especially when hot.

check to make sure the pcv hose going from the back of the engine to the PCV on the passenger valve cover doesn't have a hole in it... these trucks are famous for that... also make sure all the air filter clamps are tight and in place...
