Once, it was a muggy Tokyo summer day, I was still in my mid-20's, super-buffed and athletic, and carrying my drowsy toddler daughter in my arms on a commuter train.
An elderly Japanese woman in a worn out kimono got up and offered me her seat. I politely thanked her and tried to decline her offer, but the rest of the passengers in the car insisted that since I was carrying a child, that it was I who had "dibs" on any seat there.
One of the other commuters then gave up their seat for me, and the elderly lady, with a beneficent smile then reclaimed her seat.

I use the bus and have done so in less extreme circumstances. perhaps just for a woman or a tired person.

well if my parents werent there i shouldent i was here first,also you gotta live life find your own spot people i know its hard but yah in the past they aint got hospital and people die in age 70 and younger so your life aint bad standing even i was old or pergnant i wouldent take some ones seat its just like a person taking someones money when you beg. =p
