Drivers liscense? please answer?

im 15 and my bday is in about a month and i have had my permit for about 8months now and i just wanted to know what i need to do to get my liscense on my bday? i heard that you have to fill out some stuff but i dont know what it is or where i can get so any help at all would be good thanks.
