And for the "Incident", I have to agree with Lance some here. It's an occurrence or event that interrupts normal procedure; ie: the car wreck on freeway during rush hour on the way home from work, which makes you an extra 2 hours late to get home because people do not know how to drive. This would not be an "Accedent", it would be an "Incident".
