What happens if someone hits you from behind and you don't have car insurance?

Some lady hit the car I was driving from behind (on the left side) and her insurance company wants me to pay for it. What happens if I just don't pay it? The car was registered and insured under my friends’ name. Would she be liable for it if I don't pay it?
