Birth Certificate and some form of photo id or social insurance card also some money to pay for your permit to learn. I do not know what the prices are now and in Canada take a fifty just to be on the safe side if young under 16yrs you will need parent
with you.

Seriously get a folder, bring every document you've ever gotten from them, the test results, your report card, and any and every form of ID you can find, they need like 3 different forms. Passport is also good.

You need to take your certified birth certificate (no copies), social security card, your mom, dad, or legal guardian, your certificate of attendance from school ( and, it's only good for 30 days), and cash to pay the fee; atleast take a $20; some places take debit/credit cards but not all.
