As far as I am aware not every driver's license in the U.S. starts with a "D". I have had a license in two different states and neither of them started with a "D". One started with a "W" and the other with a "B". Maybe you could give an example of what you mean.

Varies from state to state! Here in NY there is no letter in most license numbers, in other states it's the first letter of the last name, in others still it seems totally random (much like the numbers in you license number)!
