it is a good practice to have the rotors at least machined every time you get new pads installed. This ensures that there will be less chance of premature wear on the new pads. Rotors will need to be replaced when the thickness is under what factory specs call for. Rotors in obvious need of replacement include warped, cracked, pitted, etc.There's not an easy "one size fit's all" answer to this question.

Depending on the type of driving, the area you live in (dirt, rain, snow, sand?) and type of terrain(hills, flat land).
The only way to determine the extent of brake wear is to visualy look at the rotor.
However, rotors SHOULD last twice as long as one set of brake pads.
In other words, rotors need replaceing every other brake job.
