The first thing you must remember that if a vehicle hasn't been maintain that the cost to fix things keep going up. No you don't need to.The emissions test analyzes your exhaust...hopefully you don't have any oil in your exhaust or you have bigger problems than the smog.

First of all, unless there are major problems with your car, you will not have a problem passing the emissions test. Also, it is the OIL companies that say every 3 thousand miles. It isn't necessary for 7-12 K miles, or every 6 months. The every 3 months 3k miles is a myth.
next, if you you want to burn it cleaner, get a GOOD quality additive. I don't know where you are, so i won't suggest anything. I get what is called "freedom fuel." It also increases gas mileage. You probably don't need a "tune up". Those are crap. Unless of course, your car is old and has never had anything done to it. Cars are built to last!!
