the most simple way to fail an emissions test is to have problem with the exhaust system (e.g a hole on the muffler or one of the pipes).
Adding oil to the gasoline helps to mess up CH levels, e.g. two-stroke additive oil in my opinion does it, although the smell and color of the exhaust gas will give the trick away :).
Messing with the intake air mass meter or the lamdba-probe is also a good way to alter the mixture, which will affect nearly all kinds of emission levels. E.g. try to start the car with pushing several times, this will flood the exhaust and the cats with fuel, which will bang up the lambda-probe in no time. ;D

make a hole in your catayltic converter.I believe the question should be "Why would you want to make your car fail an emission test?" If you want to retire your vehicle just don't re-new the registration and sell it to a salvage yard.
