In Ms., they check the headlights, brake-lights, blinkers(which many don't use!). They also have a device that checks the tint of the windows.(It cannot be too dark) They do not have an emissions check here.

It depends on what state/country you live in - some places don't even have them.

why does my car's engine rev while the car is idling?

They check your breaks, all your safety lights, cracks in windows, wheel tread thickness.

Is the scanner enough to detect an auto transmission problem?

Depends on the laws in your state. Here in Michigan, there was an emissions test years ago but they did away with it (now they're talking about bringing it back). In California, they do a smog test where they check hydrocarbon and carbon monoxide output as well as the California emissions equipment on the vehicle. In New York state, they do an actual vehicle inspection where they check your windshield (any cracked glass in the viewing area and you have to get the windshield replaced), all your lights and signals to ensure they're all in working order, your tires to ensure they meet safety requirements, and your fluids in some counties (if your car leaks anything, you have to get the leak fixed before they'll allow you to register your car).

Lights, turn signals, wipers, brakes, front suspension and ball joints, obvious tire wear, proper documentation (driver's license, insurance card and registration), your exhaust system for leaks, cracks in the windshield, front seat belts, and a functioning horn ... that's what gets checked, along with emissions, in New Jersey!
