How to remove a master cylinder push rod without damaging the cylinder plunger?

Question:
note the push rod has a locking clip when inserted into the plunger

Could anything cause oil cap to be blown off besides it not being screwed on tight enough?

check under the dash,near the brake.unhook the rod from the brake pedal and take the master cylinder and rod out togeather.some has a clip to hold the rod in the master cylinder.
