I had this rebuilt 10 months ago for $1800, the warranty was 12months or 12k miles. I put 14k miles on it though. What could make this slip? Lack of oil? The only other indicator, I know when they rebuilt this last time, my engine mounts seemed more worn and there has been engine vibration over the 10 months.

1) Yes, fluid could have something to do with this, but I would think a C70 would warn you of low pressure/fluid level. Check it!

3) Unfortunately, I think you got a bad tranny job. Unless you've been really derelict on checking your tranny fluid nothing should be going worng with just 14k miles on it. Go back, explain the problem, don't let them off the hook for 2k miles and get it fixed!
