A loaded caliper comes with pads and hardware included. An unloaded caliper is bare. Many manufacturers now produce a "semi-loaded" caliper, which has hardware included, but no pads. These are popular with garages who prefer to use their own brand of friction.

For what you get, the loaded calipers are the best value IF you're doing both sides. I specify this because you don't want to have a vehicle with worn pads on one side, and brand new pads on the other.
