Your forth gear is most likely the overdrive gear. It sounds like your transmission needs to be replaced or repaired. If you plan on keeping the car, then you should replace it with a refurbished one. Or, sell it and get yourself another car. Not having a forth gear is OK as long as you are not on a highway. Otherwise, your RPM will be so high that it will overheat your engine.

Try pressing the overdrive button. The overdrive position is supposed to be "on", but when you need some extra power for towing, or passing, you would turn it "off" and it will keep the car in a lower gear.
