It's a head gasket. If you want to do the job correctly check for cracks in all the combustion chambers. If you have no cracks, deck the cylinder head when you have it off. This insures that the head is flat and smooth. If the engine comes out of the van check for cracks in the block if it's OK deck the block also. At a bare minimum have a qualified automotive machine shop check all surfaces for flatness. Check the spray patterns of your injectors.
