Of course it is possible. Although the chances would be slim, there is that small percentile that states the train can go off the rails. Any vehicle that rides on rails can go off track. Maglevs, High-Speed electric, Steam, etc. all have the capability of going off track. A mechanical issue within the locomotive or in one of the cars it is pulling could be the culprit of such an issue, or an issue with track (i.e distorted rail, material on the track).
