Generally, the right of way should be given to the motorist to your right. However, problems arise because people don't all arrive at the stop at the same time. So, for example, if you arrive at the intersection, but another driver arrives just a bit later, you might be unsure of whether to go through the intersection or give the right of way to the later vehicle. I think that the safest bet is to always yield to the vehicle on the right, unless that vehicle arrives after you've started to move your vehicle through the intersection.

Because they don't know how to drive. I get really irked by drivers who come to a 4 way stop, slow down, and then proceed through the intersection with total disregard for all of the other people at the intersection. Some of them aren't sure which rules to follow. The rules of the road are technically that people go in the order they arrived. But in practice, most people go in cross-pairs, regardless of who stopped first.
