Serious accidents and dangerous occurences must be reported to your Enforcing Authority for health and safety. This will be your local council environmental health department if you are a low risk business (shop / office / restaurant etc) or the Health and Safety Executive if you are higher risk (construction / factory / hospital etc). If you need to report an accident, do it via www.riddor.gov.uk and it will be forwarded to your enforcing authority.
There is no legal requirement to record inspections, but you should be given a hand written receipt at the time of inspection by the officer for you to keep. This should state what action they intend to take following the inspection and you should keep this. Some companies make their own forms to record inspections, with just the who, when, what sort of detail on it.

The most important thing is to be on the lookout for a letter from the officer detailing what improvements you need to make and carry these out, or suitable alternatives, within the timescales that should be on the letter. You should have a designated Health&Safety officer at work and be doing a monthly H&S Audit, to idenify hazards and how they are going to be dealt with. It does depend on what type of work you are doing. Some companies may require Risk Assessments to be done in order to identify hazards.You sould also have an employee accident book and access to RIDDOR recording in the event an accident has a member of staff off for more than three consecutive working days.
